feat: inline rendering with click-to-edit
- Render PlantUML diagrams inline (no Render button)
- Hide child code block via CSS when viewing diagram
- Click diagram to reveal code block and enter edit mode
- Click away to re-render diagram automatically
- Switch build from Parcel to esbuild (minified, 82KB vs 154KB)
- Remove unused convert.ts
