Reflect the removal of PlanBackend/MemoryBackend abstractions and the addition of pkg/tools/memory and pkg/tools/task. Update tool table (12 tools, 5 servers), data flow diagram, planning lifecycle, and consumer setup example. Rewrite PLANNING.md around task_plan/task_complete.
