Removed two inline 'Dead Ends and Reversals' tables scattered mid-document. Added single comprehensive table at the end covering all reversals from all phases — including hooks, event ring buffer, abstractions, namespace merges, concurrent-by-default rejection, and every other thing that was built and then killed. |
