tools: remove file_read/file_write; fix mkfile all target

Update submodules to drop file server. Fix root mkfile: mark 'all' as :V: so mk doesn't treat it as a file target. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>
