Levi Neely f87699ac7d Unify tool servers: dissolve builtin/MCP distinction ... All tool servers now implement tools.Server equally. tools.MCPExecutor → tools.Dispatcher (unexported dispatcher), tools.Executor → tools.Dispatcher interface, tools.NewMCPServer → tools.NewServer, execute.Executor → execute.Server, file.Server added. BuildAgentEnv takes a pre-configured Dispatcher; caller registers execute and file servers before calling. builtinServer collapsed — no special "builtin" concept remains.
