unify DirStore: single type replaces FlatDir, Union, ToolStore
- New store.DirStore with options: WithReadOnly(), WithIndex(fn) - Supports single/multi dir, synthetic idx, ListDir, MkdirAll - Removes UtilStore, ExecStore, ToolStore wrapper types - toolIndex extracted as standalone function - All stores in server.go use store.NewDirStore() - SkillStore stays as-is (different storage model)
